Client Assessments CHS’s licensed professional geriatric care managers will visit privately with elder patients at their homes, to evaluate individual needs. Family members are invited to attend the two-hour meetings. As part of a routine assessment, our staff members will: • Obtain a complete history • Determine a client’s level of cognitive functioning • Conduct a depression screening • Survey the home environment for safety features • Assess diet and nutritional needs • Appraise opportunities for socialization • Obtain a list of current medications With that information, we’ll create a detailed, written care plan designed especially for each elder client. <back to services
