Patient Assessments
CHS’s
licensed geriatric care managers will visit privately with elder
patients at their homes, to evaluate individual needs. Family members
are invited to attend the two-hour meetings. As part of a routine
assessment, our staff members will:

Determine the
client’s level of cognitive functioning

Conduct a
depression screening

Survey the home
environment for safety features

Assess diet and
nutritional needs

Appraise
opportunities for socialization
With that
information, we’ll create a detailed, written care plan designed
